HOW TO DISCHARGE A PATIENT VIA CRIS:
You will need to write two “Notes” and a dictated Discharge Summary. One of these Notes (the Discharge Routine Progress Note) will generate the Discharge Order automatically.
First Note: “Patient Discharge Instructions.” (Go to Write Note and just type the word “Patient” into the free text prompt; then click the PDI.) Here you need to fill in…
Take Home Meds (I don’t click the boxes on top, just skip down to free text section, click “F6” and then select “Outpatient Medications” and reconcile the rest here.
Diet: usually “regular”
Activity: usually “as tolerated”
Follow-up appts
Other instructions: One or two-liner with why they were admitted and what the intervention was
Second Note: “Discharge: Routine Progress Note” (Just free-text “Discharge” and this will populate. This will generate the Discharge Order. You do not need a separate order.)
Leave all boxes checked on top.
Date and time of discharge
D/C to: OP-12
Special instructions: attend appt on (specific date) as scheduled (type in if you know the appt)
Problem list: Do NOT abbreviate
Outside information: brief summary of hospital course (1-liner here)
Discharge Summary DICTATION
Can only be dictated after the discharge order is in (after you write your Discharge Routine Progress Note)
Follow instructions the pamphlet you have been given
If you are like me and dislike dictation, I recommend free-texting your note in Word and saving the file. Then call and dictate a “blank template;” you can cut and paste your note in it in the ESA application afterwards. It takes a few hours to a day to generate a dictation note or template. Writing a Word document is proactive while you are still on the floor and remember your patient.
